Tuberculosis abdominal, un reto diagn??stico Utilidad de la laparoscop??a

RESUMEN
Introducci??n: la enfermedad producida por Mycobacterium tuberculosis es a??n un problema de salud p??blica mundial. A pesar de la alta incidencia de tuberculosis (Tb) la infecci??n abdominal es infrecuente y presenta dificultades para su diagn??stico oportuno. Caso Cl??nico: paciente femenina de 8 a??os de edad, llevada al servicio de urgencias con un cuadro cl??nico inespec??fico (p??rdida de peso, dolor y distensi??n abdominal), se obtuvieron im??genes diagn??sticas y ex??menes de laboratorio con resultados no concluyentes, por lo que se realizaron biopsias peritoneales laparosc??picas las cuales permitieron confirmar el diagn??stico sospechado. Comentarios: las manifestaciones cl??nicas inespec??ficas y la poca sensibilidad y especificidad de las ayudas diagn??sticas de este caso, coinciden con los resultados y reportes de otros estudios, esta patolog??a plantea un gran desaf??o diagn??stico. La laparoscopia con biopsia debe ser considerada como una herramienta diagnostica ??til cuando se sospeche Tb peritoneal.
ABSTRACT
Introduction: The disease caused by Mycobacterium tuberculosis, remains a global public health problem. Despite the high incidence of tuberculosis, abdominal infection is uncommon and represents difficulties in early diagnosis. Case report: we present a 8 years old female patient, brought to the emergency department with nonspecific symptoms (weight loss, abdominal pain and bloating), diagnostic imaging and laboratory tests with inconclusive results were obtained, so laparoscopic peritoneal biopsies were performed and confirmed the suspected diagnosis. Reviews: nonspecific clinical manifestations and the low sensitivity and specificity of diagnostic tests in this case are consistent with the results of other studies. This disease represents a major diagnostic challenge. Currently in the light of scientific evidence, laparoscopy with biopsy should be considered as useful diagnostic tool when peritoneal tuberculosis is being suspected.
